The form below is for any person worshipping outside of the Diocese of Brooklyn who wishes to serve the movement in any capacity. When this form is received, Fr. Jose will send it to the Diocesan Office for approval. No-one outside of the Diocese of Brooklyn is allowed to serve until the Diocese has approved their application. Once Fr.
